Our post-rehabilitation care is a specialized service that will assist your love ones recovering from illness or injury. We will work with their therapist to promote independence, quality of life and return to a safe and fulfilling lifestyle in the home setting.
in simple terms, our post-rehabilitation care service is designed to help our clients return to their highest level of independence at home. It is our goal to work towards recovery and improved independence.
In addition, if continuing care is needed, we will accompany and assist them to go to:
- Outpatient therapy which includes physical, occupational, or speech therapy
- Cardiac rehab
- Pulmonary rehab
- Wellness program
